Rather, value is about how well they utilize their knowledge and\nexperience to create a customized solution to meet your hearing expectations\nand your lifestyle.<br>\nAccording to HIPAA regulations,&nbsp;you have the right to restrict the uses or\ndisclosures of your information made for purposes of treatment, payment and\/or\nhealth care operations.<\/p>\n\n\n\n<ul><li>Treatment is the provision, coordination or management of\nhearing health care. For example, we may use and disclose your information to\nconsult with a third party or to refer you to other health care providers. We\nwill get your written consent prior to making disclosures outside our practice\nfor treatment purposes, except in emergencies.<\/li><li>Payment includes the activities necessary to obtain\nreimbursement for the provision of hearing health care. For example, we may\nneed to give your health plan information about treatment you received at our\npractice, so your health plan will pay us or reimburse you for the treatment.\nWe will get your written consent prior to making disclosures for payment\npurposes.<\/li><li>Health care operations include the activities necessary for our\npractice to run its business operations. If you choose to\nfile a complaint, we will not retaliate in any way.<\/p>\n\n\n\n<p>This practice is determined to protect the privacy of your\nmedical information. As we provide service to you, we create and store health\ninformation (a medical record) that identifies you. It is often necessary to\nshare or disclose this health information in order to provide treatment for\nyou, obtain payment and to conduct health care operations in our office.<\/p>\n\n\n\n<p>This Notice of Privacy Practices requires us to:<\/p>\n\n\n\n<ol><li>Keep\nyour medical records private and to provide you with this notice.<\/li><li>Update\nour privacy practices and the terms of this notice at any time, ensuring our\nnotice is effective, even for information recently obtained.<\/li><li>We\nreserve the right to make an important change in our privacy practices and\nchange this notice to that effect. You may contact us to request a new copy of\nour notice and we will make the new notice available upon request.<\/li><\/ol>\n\n\n\n<p>The following is a description of the different circumstances\nthat may require our practice to use or disclose your medical information:<\/p>\n\n\n\n<ol><li>Share\nmedical data with another provider who is responsible for your care (physicians,\naudiologists, nurses, any other health care professionals, technicians,\nstudents in health care or any other people who take care of you), make\nreferrals and\/or placing lab\/prescription orders.<\/li><li>Share\nyour health insurance plan information about a treatment you received at our\npractice when filing a claim for reimbursement or determination of benefits.<\/li><li>Provide\ntreatment communications concerning treatment alternatives or other health\nrelated products or services, unless we or a business associate receive\nfinancial remuneration in exchange for the communication in which case we must\nreceive your written authorization unless the communication is made\nface-to-face or involves gifts of nominal value.<\/li><li>Disclose\nmedical information to a medical examiner to identify a deceased person or to\ndetermine the cause of death, or for tissue donations.<\/li><li>Medical\ninformation may be disclosed if you are military personnel, either active or a\nveteran, and if required by the appropriate authorities.<\/li><li>Share\nmedical data to the public health and\/or law enforcement official whose job is\nto prevent or control disease, injury or disability.<\/li><li>Share\nmedical data with a representative from the Food and Drug Administration for\nthe purpose of reporting adverse effects stemming from defective products, etc.<\/li><li>Medical\ninformation may be disclosed when necessary to comply with Workers\u2019\nCompensation.<\/li><li>Medical\ninformation may be disclosed in response to a court and\/or administrative order\nin a lawsuit or similar proceeding.<\/li><li>In\norder to contact you for fundraising activities supported by our practice. You\nhave the option to opt out of receiving these communications by sending a\nwritten request to the privacy officer.<\/li><li>For\nmarketing purposes for which our practice or our business associates may receive\nremuneration, for a disclosure that constitutes a sale of protected health\ninformation, and in all other situations not described in this policy your\nwritten authorization will be obtained before our practice will use or disclose\nyour health information to third parties outside our practice. You have the\nright to revoke such authorization by providing our practice with a written\nrequest to revoke the specific authorization.<\/li><li>If a\nuse of disclosure is required by law, the disclosure will be made in compliance\nwith the law and will be limited to such requirements. State and federal laws\nmay be more stringent and may prohibit certain uses and disclosures identified\nabove. When another law is more stringent than HIPAA, we will follow the more\nstringent requirements.<\/li><li>To\nbusiness associates to perform functions on our practice\u2019s behalf, if the\nbusiness associate has signed an agreement to protect the confidentiality of\nthe information.<\/li><li>Share\ninformation about your condition(s), location and\/or death to family member(s)\nor your personal representative(s). Prior permission by you will be obtained\nunless in case of emergency. If we are unable to obtain permission, we will\nshare only the health information directly necessary for your health care.<\/li><\/ol>\n\n\n\n<p>You have individual rights as part of the notice of Privacy\nPractices. As a patient of Ear, Nose and Throat Associates LLC,\nyou have the right to:<\/p>\n\n\n\n<ol><li>Request\nour practice to restrict uses and disclosures of your health information.\nHowever, we are not required to agree to the requested restriction unless you\nare requesting a restriction on the use and disclosure of your protected health\ninformation to a health plan for payment or health care operations and such\ninformation pertains to a health care item or service which you paid for in\nfull and out of pocket. These requests should be made in writing to the address\ngiven in this Privacy Notice. In your request, you must tell us (a) what\ninformation you want to limit; (b) whether you want to limit our use,\ndisclosure or both and (c) to whom you want the limits to apply.<\/li><li>Be\nnotified upon a breach of any of your unsecured protected health information.<\/li><li>Request\nthat we communicate with you regarding your confidential medical information by\ndifferent means or to different locations. This request must be made in writing\nto our practice.<\/li><li>Request\nphotocopies of your medical records on file and\/or a copy of this Notice of\nPrivacy Practices. If you need a photocopy, please notify the receptionist.<\/li><li>Request\na change to your health information if you think it is incomplete or\ninaccurate. However, if the audiologist, hearing health care professional or\noffice personnel believes the patient\u2019s health information is complete and\naccurate, he\/she can refuse to make the requested changes. This request must be\nmade in writing to\nEar, Nose and Throat Associates LLC .<\/li><li>Receive\na list of all the times your medical information has been shared by our office\nor our business associates for six years prior to the request date, other than treatment,\npayment, health care operations and\/or other specified exception.<\/li><li>Request\na paper copy if you have received this Notice of Privacy Practices\nelectronically.
